The EyeMed vision plan is a program which provides coverage and discounts for eye exams, lenses and frames. The program has the flexibility of in- and out-of-network coverage through EyeMed. When you use an in-network provider, your out-of-pocket costs are lower. You are still covered if you use an out-of-network vision provider. Associates who enroll in this program pay 100 percent of the premiums via bi-weekly paycheck deductions.
When considering enrollment in the vision plan, it is important to evaluate the cost of the benefit compared with potential vision care expenses for the upcoming year.

regularly scheduled to work 32 or more hours per week are eligible.
HOW THE VISION PLAN WORKS
You have two choices when you need vision care:
• If you visit an EyeMed network provider, you simply pay a copayment for exams, and your out-of-pocket costs are lower
• If you use a non-EyeMed provider, you pay the provider for all services and supplies, and submit a claim form for reimbursement. You are reimbursed based on a fixed schedule of benefit allowances.
